For some people, flat feet cause no problems at all. However, the demands placed on the feet can change how well they tolerate that foot structure. Someone who spends long hours standing, walks extensively for work, runs, plays sports, hikes, or participates in other high-impact activities may place considerably more stress on their feet and lower limbs.

When flat feet are associated with excessive or poorly controlled foot motion, custom orthotics may help provide additional support and influence how forces are distributed through the foot and lower limb. By improving pressure distribution and supporting foot function during walking, running, or prolonged standing, orthotics may help reduce excessive stress on tissues that are repeatedly loaded during activity.

This doesn't mean orthotics can guarantee injury prevention. Injuries are influenced by many factors, including training volume, footwear, strength, mobility, recovery, previous injuries, and biomechanics. However, for someone whose flat-foot mechanics are contributing to excessive loading or recurring symptoms, addressing those mechanics may be an important part of an overall injury-prevention strategy.

This is particularly relevant for occupational demands. Healthcare workers, tradespeople, retail workers, warehouse employees, first responders, and others who spend much of the day standing or walking may accumulate thousands of steps during a workday. Even relatively minor biomechanical issues can become more noticeable when the feet are exposed to repetitive loading for many hours.

For active individuals, the same principle applies. A person may have flat feet without symptoms during everyday activities but begin experiencing arch fatigue, heel pain, shin discomfort, knee pain, or other problems when they increase their running mileage, start a new sport, or take on more demanding activities.
